Referral Procedures

Placement in Hearts of Gold Homes requires a referral form accompanied by other personal history information. The psychologist, physician, administrator, assistant administrator or owner will review the information and decide if the person is suitable to reside in the Hearts of Gold Home. Prospective consumers must “fit in” with consumers residing in the home. The final compatibility decision for placement will involve the person being placed, SDRC case manager, Hearts of Gold Care Homes owner, psychologist, and advocate and/or family member.
